Step 2: Complete a Teacher Education Program Application Packet
- Standardized Test Scores – Need PRAXIS CORE
- Praxis CORE: The following test scores are required: Reading Test (156), Mathematics (150), and Writing (162)
- You must have an official score result in hand by your portfolio due date in order to apply to TEP. An application without an official score is incomplete and, therefore, not eligible to proceed.
- It is advised that you take the exam at least 6 weeks in advance of your anticipated interview for scores to be submitted.
- If you are having financial difficulties, contact Dr. Heather Erwin (heather.erwin@uky.edu), as there may be financial assistance we can offer.
- Be on the lookout for Praxis CORE workshop dates. These workshops provide study skills to help you be successful taking the Praxis CORE.
- The Praxis CORE exams currently cost $135 for all three sections.
- Recommendations
- Minimum of 3 recommendation forms (PDF) completed
- Recommendation forms may be accompanied by an additional letter, but the additional letter is not required. One recommendation must be from a college instructor. Completed forms do not need to be in sealed envelopes.
- Character and Fitness Review (PDF)
- Disclose any ethical or legal difficulties you may have had.
- If you answer “yes” to any question on the review (except #1), go to: Character and Fitness Review or come to TEB 166. You must submit additional materials to TEB 166 prior to submitting the TEP application.

- Standards Sets Self-Assessment Worksheet (PDF)
- Transcripts
- Official or unofficial transcript from UK and official transcript only from any other college/university attended.
- If there are any schools listed on your UK transcript (including KCTCS schools and college work completed in high school), you must have a copy of the transcript from that school. (Although the # of credit hours appears on the UK transcript, the grades awarded from those courses are not listed). Only UK transcripts may be unofficial. Legible photocopies of official transcripts are acceptable. Please inquire in TEB 166 if a copy of the transcript is on file (before the deadline) and, if so, we will provide you a copy.
- Check or money order for $30.00 to the “University of Kentucky”
Once you have completed the application packet, it must be approved by the Office of Academic Services and Teacher Certification staff by the deadline stated on page 1. This office is located in 166 Taylor Education Building.
