For example, if you complete your assignment in May (spring semester), you may use your credits in the following summer, fall or spring semesters; if you complete the assignment in December (fall semester), you must use your credits during the following spring, summer or fall semesters. You must complete this form prior to each semester you plan to enroll.
- You must have completed your supervisory assignment after August 1, 2000.
- You must be admitted to the University of Kentucky Graduate School and be in good standing. For further information about admission contact Ms. Pat Bond at 859-257-4905.
Students dropping courses two weeks after classes begin will not be eligible to re-enroll for the withdrawn credit hours. The College of Education is not responsible for late fees or reinstatement fees.
To receive a waiver, your form must be received in the Associate Dean’s Office prior to the first day of classes for the semester you are enrolling in. A new application must be submitted for each semester you are taking classes.
