Teacher Certification Applications

 

Candidates who are applying for teacher certification will complete and submit the UK College of Education teacher certification forms, the Kentucky EPSB TC-1 application, and a UK transcript request form. Printed copies are available in 166 TEB, or can be printed from the links below.

Once completed, the forms should be returned to the Office of Academic Services and Teacher Certification (166 TEB).
